Director Jennifer Truong, Occupational Therapist

Jennifer is a highly passionate Occupational Therapist with 22yrs experience as a treating therapist specializing in Geriatrics . She is certified in Cardiac Rehab, Stroke therapy, manual therapy and NDT techniques.

She is also bilingual in English and Vietnamese. Jennifer has also been in the role of Director of Rehab 20/22 yrs where she is directly responsible for managing both small and large teams of therapist and developing and establishing clinical service programs. Jennifer also was recognized and received the healthcare excellence award and best rehab team in the area by City mayor in 2015.

Jennifer truly cares and treats all her patients with respect and love like they were her family. In her many years as an OT she has many success stories and has developed personal relationships with many of her patients that have become like family to her.

Maria Padilla, Physical Therapist

Maria earned her Bachelor of Science in Physical Therapy in the year 2000 in the Philippines. She spent her early years volunteering and working at a tertiary hospital that provided both inpatient and outpatient care. In addition, she was a freelance home health therapist while working for a non-profit organization that catered to the rehabilitation needs of children and adolescents with disabilities.

After immigrating to the US in 2011, she began working in a skilled nursing facility that provided both inpatient and outpatient rehab services. She decided to continue her studies, so she enrolled in a postgraduate program. In 2020, she earned a Doctorate in Physical Therapy specializing in Manual Therapy.

In August 2022, she began working for Premier Rehab LLC. She was nominated for Employee of the Year and Clinical Excellence of the Year 2022. Maria’s hard work and dedication earned her the Clinical Excellence of the Year Award in 2023. With over 22 years of experience in the field of physical therapy, Dr. Maria Alicia Padilla’s approach is rooted in evidence-based practices, ensuring each patient receives the best possible care.

Blaire Lewis, Speech Therapist

Blaire is the Speech-Language Pathologist at Oak Grove Nursing Home. She received her Bachelor of Science degree in Speech and Hearing Sciences in 2018 and her Master of Science in Speech-Language Pathology in 2021 from Lamar University.

Over the past few years, Blaire has focused her clinical skills on evaluating, diagnosing, and treating adult and geriatric patients with disorders in motor speech, language, cognitive-linguistics, swallowing and voice. She is certified in Ampcare ESP, an effective swallowing protocol that combines neuromuscular electrical stimulation with resistive exercises and restorative posture device to improve the swallow physiology in those with dysphagia.

She holds her Certificate of Clinical Competence with the American Speech, Language, and Hearing Association (ASHA), is a member of the Texas Speech, Language, and Hearing Association (TSHA).
